Warehouse Forklift Operator Certification (In-Person Class)

What's Included: 

This in-person training allows students to obtain the necessary knowledge and skills required to operate Class 1, 4, or 5 warehouse forklifts safely. Upon successful completion of the classroom theory and final written exam, students must successfully complete the hands-on practical evaluation to receive operator certification.

 

Topics include:

- Responsibilities as a user and operator

- Conducting a pre-start inspection

- Conducting a workplace inspection

- Various types of equipment

- Safe operation

- Preventing accidents and identifying hazards in operation

 

Requirements and Additional Information:

- Decrease your business' liabilities by having your supervisors and employees certified as Forklift Operators.

- Participants must be at least 18 years of age.

- All participants must wear pants and tennis shoes (or steel-toe boots). Those who arrive wearing shorts or improper shoes will not be allowed in the class and will be charged a $25 rescheduling fee. No Exceptions.

- Digital (printable) certifications are issued within 24 business hours of successful class completion.

- Hard copies of the Certification cards will be mailed to the participants within 30 days.

- Digital Certifications and Certification cards are valid for 3 years.

- Florida Training Academy will loan protective eyewear, gloves, vests, and hard hats to each of its students. Please return all of the loaned equipment back to your instructor at the end of the class.

FDLE Live Scan Background Screening for CNAs can be done at the end of your CNA class -or- you can contact our office to schedule a fingerprinting session. 

 

Note: The cost of the State examination is an additional fee of $155. Additionally, the required Level II background screening is an additional $87.50. These fees total $242.50 and are paid separately. Students can pay for their CNA Exam Prep Class first and then pay for their state examination and background screening at a later date.

           

Weekend Classes are Saturday and Sunday only for one weekend.

** THIS IS AN ACCELERATED COURSE. THIS CLASS IS NOT DESIGNED FOR ALL LEARNERS. **

 

Our accelerated class is best for students with personal care experience (ex: family members); those who have worked in other roles within the healthcare field (HHA or MA); nursing/medical students; and/or those who have family members who currently work in healthcare settings who can serve as a mentor. Students who enroll in our class must be committed to learning at least 10 skills per day and agree to complete a homework assignment during the 2-day class.

 

We are serious about educating future Certified Nursing Assistants (CNAs). We can teach skills but we cannot teach character or emotional maturity. Working as a CNA requires a level of compassion, integrity and emotional intelligence.  Therefore, we hold our students to a higher standard. We will have fun, but we must also be respectful of the time constraints needed to complete an accelerated class in 2 days. Also, we teach skills with an emphasis on compassionate care. As such, students who are unable or unwilling to act in accordance with our guidelines may be dismissed without a refund.

 

Learners who need more time should register for our  3 or 5-day CNA Exam Prep Courses.  We will not slow our classes and/or focus too much time on "1" student. Doing so is unfair to our Fast Track CNA students, who do not want to see simple skills repeated multiple times. Thank you for understanding and agreeing to our policies before registering for our accelerated Weekend CNA Exam Prep Class.

Our phlebotomy workshop is best suited for Certified Nursing Assistants, Medical Assistants, EMTs, and/or returning Phlebotomists and Nurses. The course includes video-based training, instructor-led discussions, and hands-on training.

*Note: Certification as a phlebotomist is not required in Florida. However, if you desire to become certified, the cost of the examination for National Phlebotomy certification is an additional $185. You will need proof of 40 successful venipunctures in order to take the national examination. You will not meet this requirement in a 1-day class. Therefore, we highly recommend that only actively working medical professionals, who can perform venipunctures on-the-job, take our course. We have a NO REFUND policy, so please attend your class date. If you do not complete your course, we only offer this class once every other month.

We offer flexible schedules and understand the needs of healthcare professionals. Urgent and same-day classes may be available for an additional fee of $50.

 

What You Will Need to Do:

1) Purchase and complete the Heartcode BLS Online Course (part 1) and email your printed certificate of completion to emathis@FLtraining.com. You can purchase the Heartcode BLS course directly from our website as an additional option at check-out.

2) Register for your BLS Skills Session with Florida Training Academy. Once we have received your certificate of completion, we will inquire about your availability and schedule your Skills Testing Session.

3) Mandatory documents to submit: 

  1. Please bring a printed copy of your certificate of completion from the online Heartcode BLS course to class.

4) Study before the skills testing session. Also, please be able to access your online BLS Heartcode materials. Students who are unable to recall the information learned during the online course will not pass the hands-on skills assessment. Please come to class prepared to succeed! 

5) Earn your AHA certification card by completing the BLS (adult, child, and infant) testing stations. Your BLS card will be valid for 2 years. Your Certification card(s) is/are issued on the same day that you successfully complete the BLS Skills Testing Session.

  

 

How to access your Online AHA Course (if purchased from Florida Training Academy)

The access code for your online course may be included on your class confirmation -or- it may be sent in a separate email. If you are new to the AHA eLearning platform, please create an account.  Once you have created your account or logged in, you can click your link again to activate your online course. Please do not begin your course if you did not log in first.  You will be able to view your Certificate of Completion once you complete the online course and the required survey (evaluation).

 

How to Schedule your AHA Skills Testing Session

Once you have completed AHA's Online Heartcode course, please email the certificate of completion to Emathis@Fltraining.com. At this point, we will inquire about your availability and schedule your skills testing session. If you prefer speaking to someone via phone, please call the office at (904) 551-0918. The receptionist will notify the designated AHA instructor of your request. In all cases, the skills testing session will not be scheduled until the Certificate of Completion from the online course (part 1) has been emailed to our office.

 

Urgent Skills Testing Sessions: If you urgently need to schedule a skills session, please feel free to contact us.  In many cases, we can get you (re)certified on the same day, but the urgent class fee will be charged. Become a CPR Instructor

AHA Instructor classes are held at Florida Training Academy in Jacksonville, Florida.

                            
The American Heart Association BLS Instructor Course will allow the participant to teach all American Heart Association BLS and Heartsaver level certification classes. The American Heart Association Heartsaver CPR Instructor Course will allow the participant to teach all American Heart Association Heartsaver level (CPR, AED & First Aid) certification classes. 
 
 
 
 
 
 
Prerequisites:

-Email a copy of your current American Heart Association Heartsaver CPR or Basic Life Support card to emathis@fltraining.com.

-Complete the online American Heart Association Heartsaver CPR or Basic Life Support Instructor Essentials course and bring the certificate of completion. The online Instructor Essentials course can be purchased directly from our website as an optional item at checkout.  BLS Instructor Candidates should only complete the BLS Instructor Essentials Course. Heartsaver CPR Instructor Candidates should only complete the Heartsaver CPR Instructor Essentials course. Be sure to order the correct online course: either Heartsaver CPR (laypersons) -or- BLS for Healthcare Providers (Medical Professionals). What to Expect During the Instructor Course:

-Instructor candidates will be given the Heartsaver CPR or BLS Provider Test. A score of 90% or better must be obtained. Additionally, CPR skills proficiency must be demonstrated. Students who do not pass the written or skills test will not be able to continue in the course. Instructor Candidates will also be given an Instructor Exam. A score of 84% or better is required. Please study before coming to the class.

 
 
 
The American Heart Association CPR Instructor Course is a three-part course.
  1. Part one, the AHA Instructor Essentials course is completed online at home or work. You can purchase the online AHA Instructor Essentials course directly from our site - during checkout. The course contains information on planning and preparation for courses, teaching courses, conducting written exams (if applicable), skills practice and skills testing, and keeping your instructor status current. Upon successful completion, a completion certificate is generated and should be printed out for your instructor record. The Instructor Essentials course takes between 2 - 3 hours to complete. 
  2. Part two will be held at Florida Training Academy, with Eunice Mathis, MSN, RN. Part two will cover information such as course organization, policies of both the AHA, card processing, administration of a course, advertising, equipment cleaning, changes in CPR and why they occur, skills testing, a written test, and more. This portion will take approximately 8 hours. Please bring food and drinks.
  3. Part 3 involves being monitored teaching your first class. This can be done during one of Florida Training Academy's many courses or you can be evaluated at your own facility (depending on location.) Additional charges apply for off-site evaluations. This evaluation must be done within 6 months after completion of part 2.
  4. In addition to the steps above, all participants must possess a valid/current American Heart Association CPR or BLS for Healthcare Provider certification card. Once all steps are successfully completed, the participant will become an American Heart Association BLS CPR Instructor and be able to conduct classes on their own.

Student Teaching: 

Course participants will be required to conduct an ACLS Course after successful completion of the ACLS Instructor Course. A BHES TC Faculty will monitor the student-teacher. After a successful evaluation, the student-teacher will be provided with a course completion card and two-year certification as an ACLS Instructor. 

 
Prerequisites: 

The following documentation must be completed and submitted prior to the start of class for all students:

  1. Have a current AHA BLS and ACLS card; and either your BLS Instructor Candidate status (monitoring needed) or BLS Instructor card. Submit a copy of the aforementioned information upon registration.

Heartcode ACLS Skills Testing - Advanced Cardiovascular Life Support

 

 

Are you a healthcare provider who needs to earn or renew your American Heart Association's (AHA) Advanced Cardiovascular Life Support (ACLS) certification card - but do not have the time needed to complete a traditional classroom course? If so, Florida Training Academy offers the AHA's Online ACLS courses and ACLS Skills Testing sessions in Jacksonville.

 

We offer flexible schedules and understand the needs of healthcare professionals. Urgent and same-day classes may be available for an additional fee of $50.

 

What You Will Need to Do:

1) Purchase and complete the Heartcode ACLS Online Course (part 1) and email your printed certificate of completion to emathis@FLtraining.com. You can purchase the Heartcode ACLS course directly from our website as an additional option at check-out.

2) Register for your ACLS Skills Session with Florida Training Academy. Once we have received your certificate of completion, we will inquire about your availability and schedule your Skills Testing Session.

3) Mandatory documents to submit: 

  1. Email a copy of your current AHA BLS card to emathis@fltraining.com.
  2. Also, bring a printed copy of your certificate of completion from the online Heartcode ACLS course to class.

4) Study before the skills testing session. Also, please be able to access your ACLS Heartcode materials. Students who are unable to recall the information learned during the online course will not pass the hands-on skills assessment. Please come to class prepared to succeed! AHA's ACLS Algorithms can be found here.

5) Earn your AHA certification card by completing the BLS, airway management, and mega code testing stations. Your ACLS card will be valid for 2 years. Your Certification card(s) is/are issued on the same day that you successfully complete the ACLS Skills Session.

 

How to access your Online AHA Course (if purchased from Florida Training Academy)

The access code for your online course may be included on your class confirmation -or- it may be sent in a separate email. If you are new to the AHA eLearning platform, please create an account first.  Once you have created your account or logged in, you can click your link to activate your online course. Please do not begin your course if you did not log in first. You will be able to view your Certificate of Completion after you complete the online course and the required survey (evaluation).

 

How to Schedule your AHA Skills Testing Session

Once you have completed AHA's Online Heartcode course, please email the certificate of completion to Emathis@Fltraining.com. At this point, we will inquire about your availability and schedule your skills testing session. If you prefer speaking to someone via phone, please call the office at (904) 551-0918. The receptionist will notify the designated AHA instructor of your request. In all cases, the skills testing session will not be scheduled until the Certificate of Completion from the online course (part 1) has been emailed to our office.

 

Urgent Skills Testing Sessions: If you urgently need to schedule a skills session, please feel free to contact us. In many cases, we can get you (re)certified on the same day, but the urgent class fee will be charged. Students who successfully complete Part 1 receive a certificate that allows them entrance into a skills practice and testing session. An AHA course completion card will be issued upon successful completion of all three parts following successful completion of the skills session.

Learn Hands Only CPR - Free CPR classes are offered by Florida Training Academy

 

It is Florida Training Academy's honor to provide free "Compression-Only" CPR training!!

 

The "hands-only" CPR class is a knowledge only class which does not result in a certification. However, participants will learn how to recognize a medical emergency, activate 9-1-1-, learn how to work as a team, use an AED, and perform chest compressions. The demonstrations are completed at our office, upon request.

Thank you for your interest in aligning with Florida Training Academy, an authorized American Heart Association training site. We look forward to mentoring you and helping you to grow as an instructor. Your annual instructor fees help to cover the associated administrative costs.

 

After completing the Heartsaver & BLS Instructor Course, you will not be able to purchase certification cards if you are not aligned with an AHA Training Center (TC) or AHA Training Site (TS).

                                                     

Instructor Candidate Prerequisites

Be identified as demonstrating instructor potential by showing:

  1. A strong desire to teach
  2. Excellent CPR Performance and Problem Solving Skills
  3. Able to follow directions and follow through with tasks
  4. Able to communicate effectively in front of a group
  5. Current (unexpired) American Heart Association (AHA) Provider and Instructor Card for the discipline you wish to teach
  6. Agree to teach a minimum of two courses per year
  7. Provide a completed instructor candidate application (you may download one here) 

 

If you DO NOT already have a Training Center/Site, you may request affiliation with Florida Training Academy. At which point, Florida Training Academy will complete Sections Two and Three of the application. You will also need to pay the required Instructor Fee of $50 before you are able to order cards from Florida Training Academy.

 

Monitoring and Mentoring

Each instructor candidate is required to be monitored teaching a course upon completion of the Instructor course. The monitoring process is usually facilitated by your Training Center (TC) or Florida Training Academy (FTA). The TC or FTA will not issue an Instructor card until a satisfactory monitoring session has been completed. A second or third monitoring session might be required. However, this is not a punitive measure, but as part of our mentoring process to ensure all instructors are proficient and feel completely comfortable to teach ALL BLS courses offered by the AHA.

  

Affiliating With Florida Training Academy (Babcock South)

If you are requesting affiliation with Florida Training Academy, you will be asked to sign an agreement. This agreement contains the following provisions:

  1. There is an annual fee of $50 (due annually) and payable at the beginning of the calendar year, or at your time of affiliation.
  2. You will pay $9.50 per card for all of your BLS AHA cards (Basic Life Support).
  3. You will pay $25.50 per EACH Heartsaver CPR, AED and/or CPR, AED & First Aid Card.
  4. You will pay $25.50 per EACH Heartsaver First Aid card.
  5. You will pay $15.50 per card for all of your red AHA cards (ACLS and PALS)
  6. You will agree to teach a minimum of 2 classes per year.
  7. You will agree to attend an annual Instructor Update. Advanced notice will be provided.
  8. You will agree to abide by all American Heart Association and Training Center/Site policies and procedures.
  9. You will agree to be monitored teaching a class at least once per year.
  10. You will retain all class records for a minimum of three years.

Post Instructor Training Requirements & AHA Training Center or Training Site Alignment

  1. Instructors are required to join the AHA Instructor Network. Please allow 3 – 5 business days after the completion of the AHA Instructor class for access to be granted.
  2. ALL customer issues must be addressed with Eunice Mathis and Florida Training Academy FIRST and in a timely manner. All instructors who are aligned with Florida Training Academy are to notify our office FIRST and allow us time to resolve any issues. Please allow 24 – 48 business hours for a response regarding non-urgent matters.
  3. For urgent matters, instructors will be provided with the personal cell phone of Eunice Mathis. Please note, that text messaging is the preferred way of communication with Eunice Mathis for immediate matters. Also, all urgent matters also require an adequately detailed email pertaining to the issue, contact information for the student, special circumstances, etc.
  4. Instructors may not use the AHA logo (torch) on advertisements. In addition, the AHA disclaimer must be added to ALL advertisements. The AHA disclaimer states: The American Heart Association promotes knowledge and proficiency in BLS, ACLS, PALS, and First Aid and has developed instructional materials for this purpose. Use of these materials in an educational course does not represent course sponsorship by the AHA and any fees charged for such a course does not represent income to the association.
  5. Certification cards must be paid for in advance. Order cards early. Please allow 3-5 business days for the eCards to be added to your instructor inventory.
